Confirmation Enrolment

Declaration

I / We request that my child receive the sacrament of Confirmation and confirm that I / We agree to assist them by following the preparation programme in the Grow In Love school textbook.

I / We understand this will help them to celebrate these sacraments in St. Patrick’s Parish, Skerries.

Child’s Baptismal Details

In order to receive this sacrament your child must be baptised. You will need to include a copy of their Baptism Certificate (Photo or Scan copy) when you submit this form.

Terms of Enrolment

The information you provide will only be used to facilitate the celebration of this sacrament. It will not be shared with anyone else except in the event your child was baptised in a different parish. The Confirmation details will then be shared with that other parish for inclusion in the Baptism register. The information will be retained on the Pastoral Management System for administration purposes only.

The information entered in the sacramental register is a permanent record.
